Once an Initial Needs Assessment has been completed with the Injured Party, the assigned Case Manager will draw up a rehabilitation plan that acknowledges injuries, possible barriers to recovery that need to also be addressed in rehabilitation and communally agreed goals to motivate progression in the process. This plan will outline costs for each of the suggested interventions based on previously agreed contracted prices. The rehabilitation plan is sent to the instructing parties for review and approval of funding and nothing will be instigated until confirmation has been agreed. The plan is reviewed regularly throughout rehabilitation to check progress or any required changes and an updated report is sent to instructing parties with any updated funding requests for approval.
Initial Needs Assessments are completed within ten working days of a successful contact with the Injured Party. We then provide a comprehensive report of the assessment to you within ten working days of the assessment.
Following successful contact with the Injured Party, we will complete a telephone assessment within five working days, and then provide you with the report five working days following assessment completion.
As part of our contracted agreement, you will receive a monthly Management Information report that outlines the information of every case you refer to us and the outcomes so far.
Upon receipt of your referral, an acknowledgement will be sent to within one working day.
We will contact the Injured Party within two working days of receiving a completed referral via telephone and make three attempts to get in touch with them.
