We are seeking experienced Associate Case Managers to join our industry-leading rehabilitation team on a self-employed basis across the UK.
Applicants must have a minimum of two years’ experience in Rehabilitation Case Management and, ideally, a professional background in Occupational Therapy, Nursing, or Physiotherapy. Ideally, you will have a professional background in Occupational Therapy, Nursing, or Physiotherapy, together with experience supporting injured clients and their families. You will understand the profound impact that injury can have on an individual’s life and be committed to helping clients achieve the best possible outcomes.
As an Associate Case Manager, you will be responsible for undertaking Immediate Needs Assessments and proactively managing a caseload of clients, many of whom have sustained life-changing injuries.
Using your excellent communication and interpersonal skills, you will build strong therapeutic relationships with clients and their families, supporting them to identify and achieve their goals. You will coordinate and deliver person-centred, goal-focused rehabilitation programmes that facilitate optimal recovery, independence, and return to work where appropriate.
This role offers the autonomy and flexibility to independently manage your caseload while benefiting from the support of an experienced and collaborative team. You will also have access to regular clinical supervision and guidance from a Clinical Lead, ensuring ongoing professional development and high standards of practice.
- A minimum of 2 years’ case management experience
- Experience in leading, managing and delivering rehabilitation outcomes with goal-focused support
- Excellent communication skills
- Full, clean UK driving licence
- Excellent analytical and problem-solving skills.
- Excellent knowledge across statutory and private healthcare and rehabilitation service provision in the UK
