Multi-trauma case management involves coordinating rehabilitation for individuals with multiple injuries. It requires a short to medium-term plan that aims to restore the injured person to their pre-injury function – or as close to it as possible.
We work closely with our clients and their families, instructing insurers and legal teams, and a wide range of healthcare professionals to deliver joined-up, person-centred care. This includes managing both physical and psychological needs, ensuring access to community support, and helping each person regain as much independence as possible.
What often goes unseen is the level of planning, support and care delivered by our teams – and how much of a difference it makes to the people we support.
In the past year alone, we’ve supported 353 multi-trauma cases through clinically coordinated, collaborative care.
We provide dedicated support from the moment a referral is received. Timing is essential in rehabilitation, especially when dealing with multiple injuries. Every case is carefully planned, with clinical insight guiding when and how support is introduced to best meet the client’s goals.
Our team initiates support early and provides consistent communication. In the last year:
On average, the lifecycle of successful multi-trauma rehabilitation is 63 weeks. Acting quickly helps us identify what support is needed early, and we deliver it at the right time, in the right way.
At the heart of our service is a specialist team with extensive clinical experience and a shared commitment to person-centred care.
Of our 60 Case Managers across the UK, 28 specialise in multi-trauma rehabilitation. Together, they bring nearly 200 years of case management experience and over 550 years of collective clinical knowledge.
They understand the complexities of recovery, not just from a medical standpoint, but from the perspective of the individual. This insight helps them coordinate bespoke, timely, and holistic care plans, often involving a multidisciplinary team, to ensure no part of the recovery journey is overlooked.
Our Case Managers focus on initiating rehabilitation as soon as it’s safe and appropriate to do so, adapting plans around the evolving needs of each person. Their work is guided by empathy, expertise, and a commitment to delivering truly collaborative care.
