Catastrophic Case Management

If an injury results in a long-term, if not permanent, disability and requires specialist support and rehabilitation, such as a brain injury or spinal injury, then Corporé’s Catastrophic Case Management service is needed.

As these injuries require intensive support and completely change a person’s life and independence, this specialist approach provides long-term, sometimes lifelong, management of care and rehabilitation.

Our specialist Catastrophic Case Managers oversee complex, multi-disciplinary treatment plans that address not only the immediate medical needs but also long-term physical, emotional, and social support requirements. They develop ongoing, trusted relationships with the injured person and their family, supporting them in adjusting to their new reality. This can include extensive home care, adaptations, vocational rehabilitation, and long-term psychological support.

The aim of Catastrophic Case Management is to help an injured client adjust to the changes their injury has caused to their lifestyle and help them rebuild a new life and optimum independence. This approach is provided for both adults and paediatrics due to the level of specialism required.

Multi-Trauma Case Management

When an Injured Party incurs multiple injuries from their accident, that may not be life-threatening or permanently disabling, we provide a Multi-Trauma Case Management service. These types of injuries require short to medium-term rehabilitation that aims to restore the individual back, or as close as possible, to their pre-injury function.

Our Multi-Trauma Case Managers holistically focus on both the physical and psychological wellbeing of an injured client, coordinating relevant treatment intervention and support to facilitate an optimum return to independence and work.

The types of injuries that are supported in this service include musculoskeletal injuries, fractures or mild traumatic brain injuries.

Embrace Major Incident Rehabilitation Service (MIRS) is Corporé’s dedicated approach, providing fast-responding physical and psychological support to individuals following catastrophic events.

Desktop

For more minor and less complex injuries, we provide a Desktop Case Management service that focuses on providing the same high quality clinical support but via telephone or video call rather than in-person meetings. Just like our other case management services, our dedicated Desktop Case Managers liaise with multi-disciplinary teams of healthcare professionals, coordinate treatment intervention and closely monitor progress, but provide their support and advice via digital methods to help a quicker return to function.

With this case management service, the aim is to support clients to maximise their recovery and return to their pre-injury level of function within months.

Yifat has a range of clinical experience supporting individuals with spinal cord injuries, amputations, complex orthopaedic trauma and brain injuries. For several years, she managed rehabilitation for terror attack victims. She also worked for the Ministry of Defence, overseeing the provision of orthopaedic equipment, including orthosis and prostheses, and managing amputee rehabilitation from prosthesis fitting to functional independence. As a Case Manager, she expertly manages the full assessment and rehabilitation process for adults with complex multi-orthopedic trauma, spinal cord injuries, amputations, and brain injury. She is dedicated to supporting her clients’ return to work, helping them regain their economic and professional potential and enjoy meaningful activities once again.

Rachel's clinical career has significantly developed. She began as a staff nurse in acute respiratory medicine. She then specialised in cardiac care and worked as a sister on a Coronary Care Unit (CCU), providing care to acutely unwell cardiac patients as well as those admitted with end-stage cardiac failure. Cardiac rehabilitation was an important focus to Rachel who was active in promoting a healthier lifestyle to prevent future cardiac events, working as a strong leader and implementing many positive changes to enhance patient care and outcomes. She also worked as a Community Matron which involved being a Case Manager for patients with a variety of physical and psychological conditions, including neurological, respiratory, orthopaedic and vascular illnesses. She now proficiently supports clients with brain injuries and complex orthopaedic injuries.

Pam became a Case Manager in 2019, using a creative and empathetic approach to motivate and support her clients to achieve their goals. She’s experienced at coordinating independent living trials, securing appropriate accommodation for her clients, through either statutory services or the private sector, alongside overseeing any necessary adaptations. She has excellent communication skills, in both written and verbal forms, enabling her to interact effectively with her clients, their support networks and stakeholders. Pam also has a sound understanding of the Mental Capacity Act, the Deprivation of Liberty Safeguards and extensive experience working alongside Court of Protection Colleagues.

Michelle works with individuals with complex orthopaedic, brain and neuropathic injuries and amputations. She is highly proficient in undertaking functional assessments and is also adept in supporting cognitive rehabilitation, including memory strategies and memory prompting. As an Occupational Therapist, she set up and led a community Occupational Therapy team to assist individuals with mental health conditions who live independently. Michelle has a rigorous knowledge of the litigation process and is adept at building positive relationships with all parties to support the delivery of optimal client outcomes.

Marian has been a qualified Registered General Nurse for over thirty years, mainly working within catastrophic spinal cord and brain injury units. As a Case Manager, she has extensive experience working within the legal process, providing rehabilitation for clients with spinal cord injuries, complex orthopaedic injuries and brain injury. She has also taught management of disability to clients on rehabilitation wards. She has an extensive network of suppliers of spinal and brain injury treatment, equipment and assistive technology and also special needs teachers and coordinators.

Lynne has worked in a variety of clinical settings, including General and Trauma ICU, Neurosurgical, Acute Stroke, Rehabilitation and NHS Continuing Healthcare. These roles have provided Lynne with invaluable experience of working with patients who have complex needs that require careful co-ordination and management. Since becoming a Case Manager in 2019, Lynne has gained considerable experience in managing clients with acquired brain injuries and spinal injuries, as well as multiple complex orthopaedic and abdominal injuries. She works closely with clients, families and professionals to achieve the best outcomes for her clients. She is also an Advanced Case Manager for BABICM.

Louise was a founding member of the award-winning Frailty service in a Manchester Hospital in 2014. She also has a special interest in working with clients with a background of Neurodiversity.

Loretta's main clinical role has been as a a Paediatric Nurse, but she's also worked in in emergency departments, intensive care and high dependency units. She's also been a Nurse Manager for a complex homecare provider where she set up and managed care packages for children and adults with spinal cord and brain injuries in their own homes. Loretta is experienced in completing thorough assessments and formulating bespoke rehabilitation plans to deliver holistic, person-centered rehabilitation, whilst also working as part of a multi-disciplinary team.

She believes in providing holistic support to her clients and their families from initial trauma and throughout the recovery process, focusing on client-centred care.

With Leanne's extensive experience of trauma care, she is able to motivate and empower her clients to achieve optimum outcomes from their rehabilitation. Her caseload includes complex orthoapedic injury, psychological trauma and acquired brain injury for both adults and paediatrics. Leanne also has experience as a Nurse Case Manager where she developed her skills at supporting clients through their medical and rehabilitation journey. She collaborates proficiently with multi-disciplinary teams, using effective communication to achieve the best possible outcomes in her cases. Leanne considers communication an essential aspect of rehabilitation, something she learnt from her time as a Critical Care Nurse in the Middle East where it was essential for her to learn basic Arabic to provide effective care.

Before becoming a Rehabilitation Case Manager, Lara's career began in a range of different clinical areas. She developed further rehabilitation experience when working in Australia, collaborating in a challenging behaviour multi-disciplinary team for adults with learning disabilities and supporting individuals to overcome trauma following catastrophic loss and help them focus on an optimum return to work and function. She is proficient in supporting individuals following major trauma incidents and providing complex multi-trauma rehabilitation, neurology – including brain injury – as well as amputation and pain management, including Complex Regional Pain Syndrome. Lara is a firm believer in maximising her clients' independence and quality of life.

Gabrielle found her specialism in burns and plastics through her clinical roles as an Occupational Therapist. She worked in an NHS major trauma centre and then moved on to treating admissions from intensive care and burns & plastics, trauma and orthopaedic patients, including military personnel. She then became a Clinical Specialist in burns and plastics, treating both adults and children in the Major Trauma Centre/National Burns, whilst also developing scar management and skin camouflage clinics for adults and paediatrics and advising consultants on functional burns reconstruction. Gabrielle has created highly effective rehabilitation pathways within the Midlands Burns Care Network and led service development activities across four burns services, delivering major performance improvements. She now leads the end-to-end assessment and treatment of clients with severe and catastrophic injuries.

Across her career, Emma has gained experience in a variety of clinical areas including, Neurology, Orthopedics, HDU, ITU and inpatient Neurological-rehabilitation. Whilst working in the acute sector and community environment, Emma dealt with complex orthopedic traumas and neurological cases, including acquired brain injuries, spinal cord injuries and amputations. Spinal cord injuries, as well as complex multi-trauma, are her main injury support focus now as a Case Manager, providing communicative, empathetic and sensitive rehabilitation case management in Scotland and the Scottish Islands.

Caroline is an accomplished Registered General Nurse who specialises in complex health needs. She is adept in prescribing, coordinating and overseeing complex rehabilitation programmes for spinal injury, brain injury, amputation and acquired neurological issues (e.g. brachial plexus, FND). She also has management experience, achieving a Diploma in Professional Management Studies, working in a Business Manager role operating in the NHS and as a Clinical Manager at Ealing Hospital. Caroline has also been Director of Clinical Services at Disability Essex, a charity devoted to the reablement of clients following acquired disabilities, predominantly spinal injury, amputation and acquired brain injury. Her extensive experience enables her to coordinate complex rehabilitation plans effectively and collaborate with multi-disciplinary teams to achieve optimum outcomes for her injured clients.

As a results-driven, client-centred Case Manager, Nicola is focused on helping Injured Parties return to optimum independence and work. She has worked in a variety of clinical and rehabilitation settings, including Wrightington Hospital, where she was involved in complex assessment and discharge planning for those with complex orthopaedic needs. She is excellent at managing the end-to-end planning, coordination and assessment of rehabilitation plans for complex poly-trauma, brain injury and amputation. She also has extensive experience working closely with the court of protection and is a volunteer at a Family Magistrate.

Following his qualification in Physiotherapy in 2015 and nine years completing his rotations in the NHS, which included working in a specialist amputee unit, Jamie specialised in community physiotherapy and neurophysiotherapy where he supported individuals with long-term brain injuries. This led to working extensively with clients with brain injuries and neurological disorders with a focus on improving quality of life alongside his client's personal goals. He now uses this experience to support clients with catastrophic injuries, helping them return to their own homes, using both private and statutory service intervention, and re-engage with meaningful activities.

As well as working in NHS and private services, Jo also worked in Social Services, developing adaptations and specialist equipment to increase client independence. She also completed community and hospital assessments to prevent unnecessary hospital admissions and supported clients with learning disabilities in developing daily living skills and increasing independence. Jo has extensive experience managing clients with neurological conditions, orthopaedic injuries, amputations, spinal injuries and muscular dystrophy. As a Case Manager, Jo focuses on empowering independence and has worked with individuals with learning disabilities, assisting the transition to independent living, setting goals regarding personal care and domestic activities, supervising carers and monitoring progress.

Katherine is a dedicated and results-driven Occupational Therapist whose clinical experience has been developed internationally. She now specialises in trauma & orthopaedics, elderly care, psychiatry and post-surgical rehabilitation and is highly skilled at coordinating complex discharges and ensuring smooth execution by leveraging a strong network of contacts, spanning rapid response, care agencies, district nursing and community therapy. Katherine has developed her role as a Case Manager over the years and now works as a Clinical Lead for Multi-Trauma within our team, supporting and guiding other Case Managers in their cases.

Amy developed clinical experience as a qualified nurse in Learning Disabilities since 2014. She has supported children, young persons and adults who have a Learning Disability, mental illness, complex care needs, integrating health needs and multi-trauma injuries. She also worked privately within a community setting caring for a young child who sustained a brain injury due to a motor incident. As part of this, she collaborated with a multi-disciplinary team and trained a full care team to effectively assist with rehabilitation. Following her qualification as a Physiotherapist, Charli went on to specialise in Neurological rehabilitation, working in some of the largest neuro rehab centres in the UK. She has facilitated many complex client journeys through acute care, long-term rehabilitation and discharge, with a focus on functional independence and client focused goals. She's also been able to provide supervision and training to others working in neurorehabilitation. As a person with a passion for learning, Charli has completed further training in spasticity management. She believes in developing her own knowledge to benefit her injured clients and is always willing to learn new skills and innovations to achieve optimum outcomes.

Since commencing his clinical career as an Occupational Therapist, Chris has focused particularly on neurological rehabilitation. This has enabled him to develop extensive experience in brain injury rehabilitation. Now, as a Case Manager, along with brain injuries, he also supports clients with amputee rehabilitation and spinal cord injuries. Within his professional career, Chris has also gained extensive experience leading and managing multi-disciplinary teams. This has led him to become a Clinical Lead at Corporé, but also complements his passion for adapted sports as he is also a wheelchair basketball coach.

She has a wealth or specialist orthopaedic experience, including establishing a rapid recovery program following joint replacements and spinal surgery, including setting up post-operative care, equipment requirements and ongoing rehabilitation needs. As a Case Manager, she develops long-term, holistic rehabilitation plans, identifying and overcoming barriers to recovery to help her clients return to sustainable work in a safe and timely manner following complex multi-trauma injuries.

As part of Jen's clinical experience, she spent fourteen years post-qualification as a Physiotherapist in a range of NHS, private sector and case management roles. In this time, she managed the assessment and treatment of patients with complex orthopaedic injuries, neurological conditions, spinal injuries and musculoskeletal injuries. As a Case Manager, Jen is now adept at spearheading the planning, coordination and delivery of holistic and multi-disciplinary rehabilitation packages for complex orthopaedic and musculoskeletal injuries, lower and upper limb conditions and psychological injuries.

Charlotte first gained rehabilitation experience when studying as a Sports Therapist at university. In this time, she supported many different football teams, ranging from amateur to semi-professional, providing basic maintenance for players on and off the pitch, whilst also offering first-aid assistance pitch-side. This led her to provide sports rehabilitation to professional football team, Bolton Wanderers, assisting players following their injuries from post-op through to return to play and also offering soft tissue support and sports massage. Whilst Charlotte continues to provide sports rehabilitation as a volunteer at an amateur football club, her work now focuses on assessments, planning, providing appropriate treatments, exercise prescription and strengthening rehabilitation for a range of musculoskeletal pathologies from point of injury to returning to work or function, whilst also helping long-term clients with maintenance.

Laura gained a broad range of experience as an Occupational Therapist within the NHS before specialising in Amputee rehabilitation. She has now been a Specialist Amputation Case Manager for over a decade, managing complex rehabilitation programmes for both upper and lower limb amputation, including prosthetic rehabilitation. She also worked in DMRC Headley Court, managing the rehabilitation of injured military personnel. In this time, she developed and delivered the OT service within the lower limb team, including amputee clients and complex musculoskeletal injuries. Laura is a results-driven Case Manager with considerable experience supporting individuals return to optimum independence.

Andrea is an accomplished Vocational Case Manager who supports the end-to-end delivery of impactful vocational rehabilitation and coaching. She is renowned for supporting a diverse variety of clients in restoring their professional and economic potential within optimal timeframes. In addition, she has extensive experience as a Mental Health First Aid Instructor, delivering training to a wide range of clients, including employers, on understanding and supporting the mental health and wellbeing of others. She maintains strong relationships with employability and support services across the UK and Channel Islands and she is equipped with a strong knowledge of welfare benefits. Andrea has also been involved in the 2018 revision of the Code of Ethics and Conduct in Case Management Practice for VRA UK, BABICM and CMSUK.

Lisa's nursing career involved working on a busy orthopaedic ward, assisting patients with activities of daily living and delivering post traumatic and surgical orthopaedic interventions and in A&E. This is also where she developed considerable experience with telephone triage and assessment through her work for NHS 111. As a Desktop Case Manager, this ability to work under pressure, identify solutions quickly and communicate clearly with people has been invaluable. She is proficient at developing bespoke rehab plans and making appropriate onward referrals to maximise recovery and facilitate timely, safe and sustainable return to work and function.

Specialist Features of our Embrace Major Incident Rehabilitation Service (MIRS)

  • A dedicated Crisis Management Team are assigned to all MIRS cases, with the expertise and specific training to deal with major trauma.
  • A specific telephone helpline is provided to all injured parties to access support, education and treatment.
  • Bespoke clinical assessments are provided for both physical and psychological injuries.
  • Support from our in-house Psychological Treatment team.

Case Management Assessments

Delivered as part of our Case Management services to better determine the needs and progress of our injured clients, we use specific assessment services.

Functional Capacity Assessments (FCEs)

FCE’s combine the subjective data provided by an injured client and the objective measures of a licensed clinician to determine and report functional abilities and progress during rehabilitation.

Vocational Case Management.

For individuals who are unable to return to their pre-injury employment, we provide support, advice and redirection to aid future, realistic return to work.

Activities of Daily Living (ADL’s) Assessments

ADL’s are standardised assessments carried out by HCPC registered Occupational Therapists to assess and enable injured parties to achieve a maximum level of independence with daily tasks.

Worksite Assessments

This assessment is used to identify the demands of employees’ pre-injury duties. Any modified or alternative duties are also assessed and used to develop a graded return to work program.

Ergonomic Assessments

An ergonomic assessment checks all aspects of a workstation (i.e. chair, desk height, equipment configuration) in the direct context of the employee’s body dimensions and medical condition. Any adjustments to the workstation will be made immediately and documented in a brief summary report.
